COVID Notification Template for ECE Programs + Resources


A NEW child care and school exposure template letter that can be used to notify close contacts of a case. Feel free to add the text to your letterhead and make adjustments to meet your community’s needs. Also, please see some additional topics and resources below:

Mobile vaccination request form: Child Cares, Schools and community organizations can request a mobile clinic to come to their school or location to provide COVID-19 vaccinations for individuals from 5+ and boosters for individuals 12+.

Rapid at-home testing program: Colorado is distributing free rapid COVID-19 over-the-counter (OTC) tests directly to resident’s homes.

How sick is too sick: Educational document to assist in deciding when an individual should stay home for many illnesses.

Outbreaks: Confirmed COVID-19 outbreak for all settings/events unless otherwise stated under special settings are as follows:

Five or more confirmed or probable cases of COVID-19, of which at least one case has had a positive molecular amplification test or antigen test performed by a CLIA-certified provider, in a facility or (non-household) group with onset in a 14 day period. Schools are required to report  both confirmed and suspect outbreaks to their local public health agency.
